Logbook

NOTE: Company doesn’t need to upload entire document as log book must be stored with plant item.               

  • Photo of front page of log book needs to be uploaded and example of recent checks.
  • Asset details to be displayed on evidence (Type and Model)
  • Owner name in log book to match the company owning the item of plant i.e. if the plant is hired in then the owner will be the hire company which will be different to the user/operator.                                                                     
  • Log book should be plant specific & supplied via recognised source.
  • Issue/inspection Date = Record this as it is listed in the log book
  • Expiry Date = None

Plant Mechanic/Fitter Qualifications

  • Name on uploaded evidence to match the Mechanic's name on the Maintenance Report or Service Record
  • Recognised Training Institution name to be displayed on document
  • International trade qualifications are not accepted
  • Black and white or colour copy of evidence is accepted

Accepted Forms of Evidence:

  • Certificate of Proficiency
  • Certificate of Competency
  • Certificate III
  • Trade / Tradesperson / Tradesman/ Craft / Craftsman Certificate
  • Certificate of completion of apprenticeship

  • Issue Date =Date of completion shown on the evidence (Only required to be entered if shown)
  • Expiry Date = To be recorded as the same expiry date as the maintenance / service record

Quick Hitch Compliance Documentation

NOTE: Hitch certification details to be stored with plant item.

  • Asset details to be displayed on evidence (Type and Model)
  • Evidence must be uploaded for Excavators fitted with Quick Hitches
  • Evidence needs to verify Fully Automated Quick Hitch (Semi-automatic Hitches not permitted without prior written approval from Lendlease)
  • Issue/inspection Date = Record this as it is listed on the uploaded  evidence
  • Expiry Date = None

Current Lifting Equipment Register 

NOTE: Lifting Equipment Register to be available with Plant Item performing lifting tasks.

  • Issue/inspection Date = Record this as it is listed on the certificate
  • Expiry Date = Record this as issue date + 12 months.

Monthly Pipeline Thickness Report

NOTE: Reports need to be stored with the pump
  • Must be recorded in readable / understandable format that ties in with plant item.
  • Report will indicate that the wall thickness test has been conducted, including the date and who conducted the test
  • Single wall pipes to have thickness tests available
  • Twin wall pipes cannot be thickness tested so volume of concrete pumped through them must be recorded with upper limits detailed.
  • Twin wall pipes to have volumes recorded.
  • Issue/inspection Date = Record this as it is listed in the logbook
  • Expiry Date = One off upload. Doesn’t expire


Crane Safe Green Sticker Inspection

NOTE: Crane Safe Sticker to be located on Plant Item.

  • Asset details to be displayed on evidence (Type and Model)
  • Must be CraneSafe certificate
  • Owner name on certificate to match the company owning the asset.
  • Duration as per expiry on certificate
  • Black and white or colour (green) acceptable
  • Only front of certificate needs to be shown when certificate is uploaded.
  • Issue/inspection Date = Record this as it is listed on the certificate
  • Expiry Date = Record this as it is listed on the certificate 

Pile Safe Green Sticker Inspection

NOTE: Pile Safe Sticker to be located on Plant Item.

  • Asset details to be displayed on evidence (Type and Model)
  • Must be Pile Safe certificate
  • Owner name on certificate to match the company owning the asset.
  • Duration as per expiry on certificate
  • Black and white or colour (green) acceptable
  • Only front of certificate needs to be shown when certificate is uploaded.
  • Issue/inspection Date = Record this as it is listed on the certificate
  • Expiry Date = Record this as it is listed on the certificate 

3 Monthly Inspection

Annual Inspection

NOTE: Inspection record to be stored with plant item.
  • Asset details to be displayed on evidence (Type and Model)
  • Must be completed by a competent person:
    • Professional engineer
    • NDT technician
    • Qualified tradesman
  • In most cases, evidence of competent person will be provided in the form of qualifications such as a diesel mechanic or mechanic. A letter from a business stating that all of their mechanics have qualifications is acceptable.
  • Date of inspection at least 1 year from the date the asset was first commissioned/ registered or reregistered whichever was first.
  • No non-compliances
  • Issue/inspection Date = Record this as it is listed in the log book
  • Expiry Date = 12 months from last annual inspection record.

10 Year Major Inspection (Cranes & EWP's)

NOTE: Inspection record to be stored with plant item.

  • Asset details to be displayed on evidence (Type and Model)
  • Must be completed by a competent person:
    • Professional engineer
    • NDT technician
    • Qualified tradesman
  • In most cases, evidence of competent person will be provided in the form of qualifications such as a diesel mechanic or mechanic. A letter from a business stating that all of their mechanics have qualifications is acceptable.
  • Date of inspection at least 10 years from the date the asset was first commissioned/ registered or reregistered whichever was first.
  • No non-compliances.
  • Cranes not to be older than 20 years
  • Issue/inspection Date = Record this as it is listed on the recertification
  • Expiry Date = recertification date + 10 years

Major Inspection (Concrete Pumps)

NOTE: Inspection record to be stored with plant item.

  • Asset details to be displayed on evidence (Type and Model)
  • Must be completed by a competent person:
    • Professional engineer
    • NDT technician
    • Qualified tradesman
  • Date of inspection at least 6 years from the date the asset was first commissioned/ registered or reregistered whichever was first.
  • No non-compliances
  • Issue/inspection Date = Record this as it is listed on the recertification
  • Expiry Date = recertification date + 6 years

Weed and Seed Inspection Declaration

NOTE: Inspection record to be stored with plant item.

  • Asset details to be displayed on evidence (Type and Model)
  • Must be completed by a person who is competent to conduct inspections
  • Inspection form to indicate that all parts of the plant item have been cleaned
  • Issue Date = Date of inspection
  • Expiry Date = TBA

List of OperatorsNOTE: Pegasus Administrators to check the profiles of each operator listed and confirm that they have the required Statement of Attainment competency that matches the plant item.
  • Evidence must be a list of operators who will be operating the plant item on site
  • All operators listed must be qualified to operate the specified plant item
  • Proof of operator training must be supplied (this is a mandatory requirement for all plant competencies)
  • Issue Date = Date of plant submission
  • Expiry Date = 1 year from issue date

Load ChartsNOTE: Evidence to be supplied if required by OEM.
  • Load charts to be stored with plant item
  • Asset details to be displayed on evidence (Type and Model)
  • Must list lifting point locations and their rated capacity
  • If applicable, must list boom configuration and the maximum load that can be lifted for each lifting point and boom configuration
  • If applicable, must list stabiliser requirements
  • If applicable, must list deductions for attachments (e.g. a bucket or quick-hitch devices)
  • Issue Date = None
