Period of employment with the current company 6. Experience in years/months in trade 7. Experience in years/months in the mining industry 8. Details of experience in tasks to be performed onsite 9. Details of training that has been provided by the employer with regard to work procedures, risk management, hazard management, the company's WHS safety management system and employee obligations 10. A list of equipment where maintenance experience exists for applicant 11. Applicant performance relating to accidents or incidents 12. Date/s of current NSW Order 41/41 medical & Chest X-Ray 13. Level of Supervision required for applicant 14. Will Hot Work be performed by applicant? Yes/No If yes, supply MDG25 Awareness Training qualification
