Medical.-.Fitness For Duties   Assessment

MUST include this detail:

• EMPLOYEE is employed as a JOB TITLE at COMPANY and has been in the position for NUMBER years. In total EMPLOYEE   has NUMBER years’ experience as a JOB TITLE.  EMPLOYEE has been medically assessed   and and is currently fit to perform the work associated with this role for Illawarra Coal. COMPANY will inform the Illawarra Coal representative of any change to EMPLOYEE’s fitness to undertake this work.

  
• Issue Date = Date of Letter
  
• Expiry Date = 100 years (unless a review date is listed then enter the expiry as this date)

Medical.-.Functional Assessment

• Functional assessment for the role being undertaken
• Name on assessment to match the person registered
• Must be current – within 12 months
• Must be completed correctly including  the name of the person, the date, the doctor’s signature and medical centre   stamp/details.
• Doctors signature and date and stamp mandatory

• Issue Date = Date of Assessment completion
• This will not expire (unless otherwise specified)

Medical.-.Order 41 Coal Board   Medical

•   Name on medical to match the person registered
•   All Medicals are sent to site for approval,   either prior to an induction or when medical competency update is   entered
•   Must be current
•   The medical must be completed correctly including   the name of the person, the date, the doctor’s signature and medical centre   stamp/details.
•   Doctors signature and date and stamp mandatory
•   Only full medicals are uploaded and verified
•   Medical will expire 3 years from date of   examination (unless otherwise specified)

  
• Issue Date = Date of examination
  
• Expiry Date = 3 years (unless a review date is listed then enter the expiry as the earliest expiring date

Medical.-.Order 41 Coal Board X-Ray

•   Name on medical to match the person registered
•   All Medicals are sent to site for approval,   either prior to an induction or when medical competency update is   entered
•   X-Ray component of this medical Must be current
•   The medical must be completed correctly including   the name of the person, the date, the doctor’s signature and medical centre   stamp/details.
•   Doctors signature and date and stamp mandatory
•   Only full medicals are uploaded and verified
• X-Ray will expire 6 years from date of examination (unless otherwise   specified)

  
• Issue Date = Date of x-ray
  
• Expiry Date = 6 years (unless a review date is listed then enter the expiry as  the earliest expiring date
