Coal Services Generic Induction - WAMBO CABA Card

• Name to match registered person
• Required as evidence that a Mines Rescue Induction has been completed – MUST be current
• Can accept Front of card only 
• Digital CABA cards are accepted as evidence for Wambo

Mines rescue attendance sheet also accepted. The sheet must include the following;

• Must be on Coal Mines rescue template
• Must be current within 6 months
• Must state in the course name must include self-rescuer/self-escape training
• Must state the workers name, company, DOB and signature to ensure accurate.

NOTE; The CABA must be provided for the initial induction. Once the CABA has expired, the worker must speak with their site contact who may provide an exemption. Exemption email must be provided by an approved employee - Kurt Bereza, Susan Adamson, Victoria Hellyer or Steven Williams.

• Issue Date = Self Escape issue date
• Expiry Date = 6 months from date of issue
