Generic.Govt Licence/Authority.Proof of Identity

• Name on the document to match the registered person
• Copy must be clear and legible
• Can be in colour copy OR black and white copy

• The Proof of Identity must be an Australian State or Federal Issued Proof of Identity.

• Must be photo ID

• If the form of ID is foreign / international, it will need to be sent to site for approval 

  Examples include:

• Drivers Licences

• Passports

• Proof of Identity cards etc.

• Onsite Track Easy Card

•  Record any Issue and / or expiry date/s which are shown on the document as the issue and expiration date/s within Onsite
