If a Sole Trader: the name on Licence should match the person registered
If a Partnership: the name on the Licence should match one of the Partners name registered in the system
If a Company: the name on the licence should match the companies registered name

Gasfitting licence (with relevant class) to be listed
Either Black and White OR Colour copy is accepted
Only the front of the licence needs to be displayed within uploaded file
Must be issued by the relevant government appointed organisation logo must be present
Must be a NZ issued licence – International Licence not accepted

Issue Date = Start date if displayed or enter as yesterdays date

Expiry Date = as shown on Licence

NOTE:  Evidence of the licence renewal from the relevant State licence lookup and payment receipt (example doc provided). This needs to show

• Licence number,
• Name of Company
• Start and expiry date (needs to show that it is current)

This can be approved for a period of 60 days from the receipt date.

• Issue Date = Start date if displayed or enter as yesterday’s date
• Expiry Date = as shown on Licence
