• The DNSP ESI Authorisation Declaration is to be completed (see example document)
• Applicants name to to match the Onsite worker being approved for authorisation
• Must have Substation Entry ticked
• Declaration must have the Verifier’s Details completed

• Issue Date = Date completed by verifier
• Expiry Date = 12 Months from date completed by verifier

• The DNSP ESI Authorisation Declaration is to be completed (see example document)
• Applicants name to to match the Onsite worker being approved for authorisation
• Must have Essential Energy Zone Substation Entry and Supervision ticked
• Declaration must have the Verifier’s Details completed
• Accepted also is a spreadsheet from Essential Energy - This will show Essential Energy as the Organisation, Given Name, Surname, Completion date and course details

• Expiry Date = 12 Months from date completed by verifier

• Completed Date

• Issue Date = Date completed by verifier
• Expiry Date = 12 Months from date completed by verifier

• The DNSP ESI Authorisation Declaration is to be completed (see example document)
• Applicants name to match the Onsite worker being approved for authorisation
• Must have SHL - Operator ticked 
• Declaration must have the Verifier’s Details completed

OR

A document from Snowy Hydro system that has:

• Title: CoW Location Authority Online
• Applicant Name – matched to Onsite worker being approved for authorisation
• Completed Date

• Issue Date = Date completed by verifier
• Expiry Date = 12 Months from date completed by verifier
