      **In the case that a positive
      D & A test is received drug and alcohol test result is supplied, laboratory confirmation that the drug which has been detected is consistent with declared medication must be submitted submit with the D & A paperwork.drug and alcohol test result paperwork - All paperwork must then be supported by a declaration by the company too (all documentation should be included within the one upload); the declaration must:

      • Be presented on a company letterhead
      • Be signed by a senior company representative, e.g. supervisor, HR representative, return to work coordinator
      • Be dated within the last 3 months
      • State that the employee’s drug and alcohol screening paperwork has been approved by the company
