| - Inductee name and date of birth to be clearly shown on the medical
- The employer listed on the medical must match the persons employer
- Must be stamped and signed by the medical practitioner
- Must be a valid QLD Section 4 certificate
- Name of mine must be either state ‘Crinum’ or ‘Various’
NOTE: Any CBM with a restriction must have a Mastermyne Sojitz Underground Medical Management Plan accompanied with the medical and referred to the Crinum HSECT Coordinator for approval The new health assessment provided by ResHealth is the equivalent to the paper-based Section 4 and will become mandatory from 1 April 2023. - Section 4 Coal Board Medical OR Reshealth can be accepted
- Reshealth medical must have Health Assessment report reference number listed

- Issue Date: Date of the CBM is the date of the examination (section 4.1h)
- Expiry Date: As per the following instruction (unless the medical has a specified 'review' date, as this date would then become the expiry date)
