- The employer listed on the medical must match the persons employer
- Must be stamped and signed by the medical practitioner
- Must be a valid QLD Section 4 certificatehealth assessment provided by Resources Safety & Health (ResHealth)
- Name of mine must be either state ‘Crinum’ or ‘Various’
NOTE: Any CBM with a restriction must have a Sojitz Underground Medical Management Plan accompanied with the medicalThe new health assessment provided by ResHealth is the equivalent to the paper-based Section 4 and will become mandatory from 1 April 2023. - Section 4 Coal Board Medical OR Reshealth can be accepted Reshealth medical must have Health Assessment report reference number listed
