  • Acceptable training documentation should include the following:

    • Subject/Topic of the Training

    • Instructor Name or Name of person(s) performing the training or evaluation.

    • Employee/ Operator Name

    • Training / Assessment Date

  • Accepted type/s of evidence include:

    • Employer Certification

    • Valid License (By Class/type)

    • Other valid Operator Evaluation/Re-evaluation document

    • Refresher Training can be documented based on the method of instruction.

Issue Date = Date of training completion

Expiry Date = 5 years from date of training completion

Operator Crane.Certificate.Crane Operator Certificate

Crane Operator’s License; in addition to the Operator’s License there is a lift plan that would be required as well as a working at heights permit. This requirement would only apply after the company has accepted the job and arranged with their host to be on site. The host would have the responsibility to ensure all Forms and Permit requirements have been fulfilled.

Front of License:

  • Crane Operator’s License should display the OEP (Operating Engineers Certification Program ) Seal in the middle of the card

  • Crane Operator’s License may state the Labor Name

  • Crane Operator’s License should display a photo of the Operator on the License

  • Crane Operator’s License should state whether it is Nationally Accredited

  • Crane Operator’s License should have an NDCA seal

  • Crane Operator’s License should have a signature of the Operator under the photo and a signature from the Certifying Company representative

  • Crane Operator’s License should state the crane types the Operator is certified to operate such as BT (Boom Truck Crane) ; LB (Lattice Boom Crane) ; TB (Telescoping Boom Crane), etc. that the Operator has been trained and is qualified to operate

  • Crane Operator’s License should state the expiration date x/xx/xxxx

Back of License:

  • Crane Operator’s License may have Crane Operator’s name in middle of license

  • Crane Operator’s License should have the Registration Number xxxxxxx or Certification Number xxxxxxxxxx listed

  • Crane Operator’s License should state the types of cranes() Operator is qualified to operate

  • Crane Operator’s License should display an expiration date which may be written as e.g., Expiration Date x/xx/xxxx or Issued: xx/xx/xxxx Expires xx/xx/xxxx

  • Crane Operator’s License may have the IUOE (International Union of Operating Engineers) Seal

  • Crane Operator’s License may state the program name: e.g., Operating Engineers Certification Program or National Commission for the Certification of Crane Operators on bottom of card

  • Crane Operator’s License may have address and telephone number listed on Certification Program on bottom of card

Expiry Date = As shown on the licence

Operator Excavator.Awareness.Trenching & Excavation User Training Awareness

Acceptable training documentation should include the following:

  • Subject/Topic of the Training

  • Instructor Name (for Safety Meeting/Toolbox Talk) OR Training Company Name (for certificate / refresher)

  • Employee First Name and Last Name

  • Employee Signature (Only required for Safety Meeting/Toolbox Talk)

  • Date

Training documentation can be in the form of a:

  • Certificate - Either issued internally by the employer company or by third party training organization

  • Safety Meeting/Toolbox Talk - Issued internally by the employer company

  • Refresher Training - Either issued internally by the employer company or by third party training organization

Issue Date = Date of training completion

Expiry Date = 12 months from date of training completion
