Certificate.Statement of Attainment.Standard 11 - Surface Operations

Acceptable evidence:

  • Statement of Attainment
  • STD 11 Card (Green Card or 'issued on achievement of competence in:')
  • Middlemount Coal "Induction Process Exemption Request" form


  • Name to match the person registered
  • Black and white or colour acceptable


If Statement of Attainment or STD 11 card uploaded:

  • All 6 of the following units must be listed: RIICOM201, RIIGOV201, RIIOHS201 (now replaced by RIIWHS201), RIIRIS201, RIIERR205 and RIIERR302


If Middlemount Coal "Induction Process Exemption Request" form uploaded:

  • Applicant's name to be listed and match the person registered
  • Exemption Request to state Standard 11 (or S11)
  • Exemption Period to be completed
  • SSE / SSE Delegate name, signature and date to be completed
  • Issue Date: as per the issue date identified on the Statement of Attainment or the ID card
  • Expiry Date: five (5) years after the issue date identified


If Middlemount Coal "Induction Process Exemption Request" form uploaded:

  • Use the exemption period as the issue and expiry dates

  • Statement of Attainment

  • All 7 of the following units must be listed: RIICOM201, RIIGOV201, RIIOHS201 (now replaced by RIIWHS201), RIIRIS201, RIIERR205, RIIERR302 and RIIERR203


If Middlemount Coal "Induction Process Exemption Request" form uploaded:

  • Applicant's name to be listed and match the person registered
  • Exemption Request to state Standard 11 (or S11)
  • Exemption Period to be completed
  • SSE / SSE Delegate name, signature and date to be completed
  • Issue Date: as per the issue date identified on the Statement of Attainment or the ID card

  • Can be either the Statement of Completion Certificate OR the STD 11 Card (Red Card, Partial Completion or 'issued on partial completion of:')
  • Middlemount Coal "Induction Process Exemption Request" form


  • Name to match the person registered
  • Black and white or colour acceptable


If Middlemount Coal "Induction Process Exemption Request" form uploaded:

  • Applicant's name to be listed and match the person registered
  • Exemption Request to state Standard 11 (or S11)
  • Exemption Period to be completed
  • SSE / SSE Delegate name, signature and date to be completed
  • Issue Date: as per the issue date identified on the Statement of Attainment or the Partial ID card
