- Statement of Attainment in Work Safely at Heights (RIIWHS204 or RIIOHS204) to be listed
- Competency unit must match exactly with the exception of the version (A, B, C etc)
- Must be Issued by an RTO with Nationally Recognised Training logo
- Black and white or colour is acceptable
- Scanned document to be clearly legible

- Expiry date to be entered as 2x year from the issue date
