| Quality.Trained.Concrete Slump Testing Record of Training | - Applicants name shown on document must match the applicants registered name in Onsite
- Training record is not required to be issued by a Registered Training Organisation (RTO) – internally issued documentation can be accepted
- Record of Training must include the following:
- Specify Concrete Slump Testing and the testing methods
- Assessor full name, signature, date and qualification/s
- Competent (C) result
- Assessment date
- Black and white or colour copies of certification is accepted
- Must be clear and legible

- No expiry date is required to be recorded
