- OHS Construction Induction to be listed
- Only the front side of this card is required to be uploaded.
- To be issued by the relevant / matching state authority:Workcover/ Safework / Worksafe
- Statement of Attainment Accepted.
- No expiry
- Black and white or colour copies accepted
- MACH Energy will accept the temporary white card evidence; whereby the individual receives a letter / statement (similar to a DL renewal or DL replacement) saying the replacement card will be issued within 60 days.

- Full Name must be shown
- DOB must be shown
