Drug and Alcohol Testing.Test.14 Days Valid

• Name on test page to match the person registered
• Both drug and alcohol testing results need to be displayed with a negative result.

• Issue date to be recorded

• Valid for 2 weeks / 14 days from date of issue

Drug and Alcohol Testing.Test.30 Days Valid

• Name on test page to match the person registered
• Both drug and alcohol testing results need to be displayed with a negative result.

• Issue date to be recorded

• Valid for 1 month / 30 days from date of issue

Drug and Alcohol Testing.Test.7 Days Valid

• Name on test page to match the person registered
• Both drug and alcohol testing results need to be displayed with a negative result.

• Issue date to be recorded

• Valid for 1 week / 7 days from date of issue

• Inductee name and date of birth to be clearly shown on the medical
• Must be stamped and signed by the medical practitioner
• Section 4 is the mandatory section
• Role listed on medical must be the same / similar to that selected in the portal

• Duration = 5 years
• Issue date = date of examination (EMO date and not the NMA sign off date)
• Expiry date = as per duration or review date listed on medical
• Restricted medicals will be sent to site for approval

Coppabella:

• Issue date  is date of Examination by EMO
• Expiry is 5 years from the date of examination UNLESS the person is over 40 years of age which then becomes a 1 year validity
• must be clearly scanned and legible

• Name to match
• Issued by any CS Health provider within the last 3 years

• Duration = 3 years
• Issue Date = date of examination
• Expiry Date = as per duration unless stipulated

• Inductee name and date of birth to be clearly shown on the medical
• Must be stamped and signed by the medical practitioner
• Section 4 is the mandatory section - is it different for Emergency Response Team ?
• Role listed on medical must be the same / similar to that selected in the portal

Coppabella:

• Issue date  is date of Examination by EMO
• Expiry date is 2 years from the date of examination UNLESS the person is over 40 years of age which then becomes a 1 year validity OR there is a review date which then becomes the end date
• must be clearly scanned and legible

• Restricted medical will be sent to Millennium Paramedics at mil_inductions@peabodyenergy.com
• If restricted medical is rejected Paramedics will communicate to the contractors

• Must be formatted on Coal mine health assessment report site access or site access plus periodic medical assessment detailed certificate
• Completed medical management plan form to be included within upload. Documents can be viewed and downloaded by clicking HERE
• Does it have to be by a certain training provider?
• Expiry
• Is black and white or colour acceptable?
• Does a particular logo etc. need to be present?

Verification check is as follows:

• Name of the inductee must be clearly shown on the medical
• Signed by the doctor
• Issue date – ensure the start date is the date the medical was completed
• Expiry is to be 5 years from assessment date shown on medical unless advised otherwise in the duration restriction section of the medical
