  • Applicants name to be shown on document
  • Applicants name on document must match cardholders registered name (However shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
  • Procedural Competency Assessment Tool – Generic QDS attachments to be uploaded
  • The following sections must be completed:
    • Candidates Details
    • Assessor Check and Signoff
    • Candidates Acknowledgement
    • Authorisation / Appointment – To Operate
    • Practical Assessment
  • Issue Date – to be recorded as the authoriser sign off date
  • Expiry Date – 5 years from issue date
