- Evidence must state completion of Powered Industrial Truck or Forklift Operator
- Training documentation can be in the form of:
- Certification: The certification provided by the employer must include the following information:
- Operator name
- Training date
- Evaluation date
- Name of person(s) performing the training or evaluation - Valid License (By Class/type)
- Refresher Training can be documented based on the method of instruction
- Safety Meeting or Operator Evaluation/Re-evaluation document
