• Black and white or colour is acceptable

• Name on certificate to match the person registered

• Statement of Attainment in to be listed on document - Statement of completion will not be accepted by any site except Yarrabee if it is completed within 6 months

• All statement of attainments given 5-year duration form issue date
• Will not be accepted without 5 mandatory RII’s –

Site.-.Cameby Downs Contractor Approval Form

• Name on Approval Form to match person registered

• Must be specific site approval form

• Eg: ‘Cameby Downs Approval Form’
• Must be signed and dated by companies site contact from Cameby Downs
• Expiry to be listed as expiry of induction (2 years from induction date)

• Issue and expiry date to be recorded
• Signed name to be recorded in comments box

Site.-.Eastern U/G Region Contractor Approval Form

• Name on Approval Form to match person registered

• Must be specific site approval form Eg: ‘Eastern Region Approval Form’

• Must be signed and dated by companies site contact
• As this generic induction covers sites Ashton, Austar & Donaldson person must have site contact from each specific site sign form if they intend on going to all 3.
• If person is only attending 1 site (e.g: Ashton) They only need signature of Ashton site contact.
• Expiry to be listed as expiry of induction (3 years from induction date)

• Issue and expiry date to be recorded

• Signed name to be recorded in comments box

Site.-.Moolarben Contractor Approval Form

• Name on Approval Form to match person registered

• Must be specific site approval form

• Eg: ‘Moolarben Coal Approval Form’
• Must be signed and dated by authorised Moolarben site contact
• Expiry to be listed as expiry of induction (3 years from induction date)

• Issue and expiry date to be recorded
• Signed name to be recorded in comments box

• Induction date to be listed on document
• Must be spreadsheet/email from specific Site Contact
• confirming person’s current induction
• Document to be checked against the spreadsheet Pegasus have to confirm that these are true and correct.
• Approval required for document Pegasus are unsure of validity

• Issue and expiry date to be recorded

• Must be spreadsheet/email from specific Site Contact
• confirming person’s current induction
• Document to be checked against the induction spreadsheet Pegasus have to confirm that these are true and correct.
• Approval required for document Pegasus are unsure of validity
