Resource.Management.ASIC Registration

• Document to show Business Name, Record of Registration date and next renewal date
• Business Name to match that registered in the portal
• Duration as per ASIC Registration Certificate
• Must be current
• Must be issued by ASIC (Australian Securities & Investments Commission)
• Business Name and ABN needs to match. If it doesn't match, government certified evidence must be provided to link the ABN or business names together. This is ideally a letter or download from ABR showing the linkage or a Trust Deed.

• Issue and expiry date to be recorded

Resource.Management.Company Director Secretary

• Must be printed, all fields completed, signed, scanned in and dated
• Must match the companies name in the system or a contractor representing the company  
• The ABN must match

•  Date on form to be reflected as issue date

Resource.Management.Legal Representative

• Must be printed, all fields completed, signed, scanned in and dated
• Must match the companies name in the system or a contractor representing the company  
• The ABN must match

•  Date on form to be reflected as issue date

Resource.Management.Partnership

• Must be printed, all fields completed, signed, scanned in and dated
• Must match the companies name in the system or a contractor representing the company  
• The ABN must match

•  Date on form to be reflected as issue date

Resource.Management.Private Company Owned by Company

• Must be printed, all fields completed, signed, scanned in and dated
• Must match the companies name in the system or a contractor representing the company  
• The ABN must match

•  Date on form to be reflected as issue date

Resource.Management.Private Company Owned by Individual

• Must be printed, all fields completed, signed, scanned in and dated
• Must match the companies name in the system or a contractor representing the company  
• The ABN must match

• Date on form to be reflected as issue date

Resource.Management.Sole Trader

• Must be printed, all fields completed, signed, scanned in and dated
• Must match the companies name in the system or a contractor representing the company  
• The ABN must match

• Date on form to be reflected as issue date

• Must be a current Australian Public / Product Liability document
• Limit of liability is a minimum of $10 million for Australia.
• Otis will accept special and general liability insurance as long as it mentions that it covers Public Liability insurance within the policy
• Must match the companies name in the system
• The ABN (Australian Business Number) must match
• If the amount shown on the document is not listed as an Australian Dollar amount (e.g. US or EURO), use a currency converter to check the amount - if the conversion at the verification processing time meets the required Australian Dollar amount the document can be approved.  In the Comment Field please enter the confirmation that the currency converter process has been completed e.g. ‘Currency Converted and meets the AUD minimum amount required’.

• Must be a current New Zealand Public / Product Liability document
• Limit of liability is a minimum of $10 million for Australia.
• Otis will accept special and general liability insurance as long as it mentions that it covers Public Liability insurance within the policy
• Must match the companies name in the system
• The NZBN (New Zealand Business Number) must match

• Issue date to be recorded

• Must be a current professional indemnity insurance
• Limit of liability is a minimum of $10 million for Australia and NZ
• Must match the companies name in the system
• The ABN must match

• Issue date to be recorded

Insurance.Workers Compensation.ACC - Primary

• Certificate of Currency for ACC – Primary Workers compensation insurance
• Name of insurer
• Policy Number
• Expiry date
• Company name and ABN must match the details registered in Onsite Track Easy.
• Note: All old SA Workers Comp certs that do not have an expiry listed on the document need to have an expiry of 30.06.2016 recorded as the expiration within Onsite (Unless advised otherwise by site). Following this date all companies will be issued the new version of SA Workers comp which will show a 1-year duration and be called ‘Certificate of Registration’.

• Certificate of Currency for ACC – Secondary Workers compensation insurance
• Name of insurer
• Policy Number
• Expiry date
• Company name and ABN must match the details registered in Onsite Track Easy.
• Note: All old SA Workers Comp certs that do not have an expiry listed on the document need to have an expiry of 30.06.2016 recorded as the expiration within Onsite (Unless advised otherwise by site). Following this date all companies will be issued the new version of SA Workers comp which will show a 1-year duration and be called ‘Certificate of Registration’.

• Certificate of Currency for ACC – Tertiary Workers compensation insurance
• Name of insurer
• Policy Number
• Expiry date
• Company name and ABN must match the details registered in Onsite Track Easy.
• Please record certificate validity dates
• Note: All old SA Workers Comp certs that do not have an expiry listed on the document need to have an expiry of 30.06.2016 recorded as the expiration within Onsite (Unless advised otherwise by site). Following this date all companies will be issued the new version of SA Workers comp which will show a 1-year duration and be called ‘Certificate of Registration’.
