• Must be issued by the relevant states Government appointed organisation and the organisation logo must be present
• Must be a QLD QBCC issued licence

• Issue Date = Start date if displayed or enter as yesterdays date
• Expiry Date = as shown on Licence 

If interim form of evidence such as a screenshot from the online QBCC portal is supplied, this may only be accepted for a time period of two months to allow time for the physical licence card copy to arrive in the mail.
