  • Must complete Schindler questionnaire.

  • May use Schindler registers, or may submit own template

  • All questions to be answered.

  • Any question with a 'yes' answer, must have attached supporting documentation

    • Black and white or colour copy can be accepted
    • No issue date required
    Date 1 year from the day of processing
    • Expiry Date = 01/01/2041

    • For Companies own template - signature is not required

    Date 1 year from signing to be recorded as expiry date within Onsite
    • This certification does not expire however for the purpose of Onsite entry, please enter the date which is calculated to be 30 years from the issue date
