| - Induction Approval Form to be completed and signed off by site
- In order to receive an induction approval you will need your Orica contact to email their Orica Manager with the following information:
- Contractor company name (exactly as it appears in the Pegasus Client Portal)
- Contractor company contact
- Name of individual contractor[s]
- Level of access required for each contractor (L1 –L4, the same as the old induction forms)
- You will receive an email with this approval once it is completed to be uploaded into portal.

- Issue date = date of authoriser sign off on form
- Expiry Date = 1 year from date of issue
