NOTE: APPLICANTS YOUNGER THAN PHOTO ID AGE Some employers have employees that are younger than the state-based age requirements to obtain a proof of id card In this situation, a student identification card or a birth certificate can be accepted with a Statutory Declaration (stating the individual is who they say they are) in lieu of the government photo identification card. - Name on form of Photo ID must match the name entered in Onsite
- Date of Birth must match the name entered entered in Onsite

- If International ID is not in English you will also need to upload an official translation document provided by a professional authorised translator - An authorised translator can be found by searching the NAATI Database: NAATI - National Accreditation Authority for Translators and Interpreters Ltd

or
Adult Proof of Age Card
- ID must be uploaded in color
- ID must be Valid (date must be current)
- f the ID uploaded is not valid - one of the other 3 forms of acceptable identification will be required to be uploaded.
- BOTH sides of drivers licences must be displayed within the uploaded file.
For all other forms of evidence, only the front of the card needs to be displayed
- The address shown on the ID (if shown) needs to match the applicants address in Onsite (If Australian issued ID is uploaded)
The expiry date is to be recorded as one of the following (whichever date is the soonest):
- The date of expiry which is shown on the uploaded evidence OR;
- The date which is calculated to be 2 years from the booking submission date
