| APPLY OR PROVIDE FIRST AID | - Applicants name to be displayed on uploaded evidence and to match applicants registered name (however shortened versions of names such as 'Chris' for 'Christopher' is accepted)
- Evidence must state First Aid Certificate
- Must be issued by Registered Training Organisation (RTO)
- Black and white or colour is acceptable
| - Issue date to be recorded as shown on the evidence
- Duration: 2 yearly (Align with Induction renewals) Expiry: 2 years from the issue date on the evidence unless only a 1 year card/ticket, then yearly
