- Front and back required if actual licence is supplied
- Must be issued by the relevant states Government appointed organisation and the organisation logo must be present
- Must be NSW issued licence paperwork
| - Issue Date = Start date if displayed on licence paperwork or enter as yesterdays date
- Expiry Date = as shown on Licence paperwork

- Document for original license matches registered business name
- Receipt has listed – SafeWork NSW D/G-Exp Licence Application
- Unique Reference No is listed

- Expiry Date = 6 Month from expiry date of current licence
