SF00General and compliance plate photosGeneral photographs means digital photographs of the plant item normally taken from all sides, which clearly show the condition of the plant and demonstrate the existence of visible compliance requirements.  These photographs should be of a suitable size and image quality to permit enlargement on a computer screen without significant loss of image quality through pixellation, etc.

Compliance plate photographs shall mean digital photographs of all manufacturer's information plates, identification plates, and compliance information plates attached to the plant item or equipment.  Photographs of compliance plates must:
a. be taken in landscape mode and the plate must fill the width photo image, or be cropped to the width;
b. be taken such that all text on the information plate is clear and legible; and,
c. be taken at a suitable size and image quality (resolution) to permit enlargement on a computer screen without excessive loss of image quality or readability through pixellation, etc.

SF01Design Risk AssessmentA formal risk assessment document that considers safety in the design phase of plant.  The design risk assessment could take many forms including a Safety in Design Report from the plant designer (or OEM), HAZOP study, Design FMEA, or one of the other design phase risk assessment methodologies that are described in ISO/IEC 31010. Refer to SafeWork Australia Safe Design of Plant Code of Practice and Safe Design of Structures Code of Practice.

SF02Operation and Maintenance Risk AssessmentA formal documented risk assessment focussing on the hazards associated with the operation and maintenance of the plant or structure, and identifying controls to prevent or mitigate risks.

SF03Fire Risk AssessmentA formal documented Fire Risk Assessment that complies with the requirements of AS 5062, including the assessment of fuel loads.

SF04Hydraulic Risk Assessment (MDG41)A formal documented fluid power risk assessment focussing on fluid power hazards identified in MDG41.  If these hazards are assessed in an accompanying design risk assessment or operations and maintenance risk assessment document, then this requirement is waived. This should be noted on the applicable Site Access Approval Form, Approval to Enter Conditional Service Form, or Approval to Enter Service Form.

SF05Registration Document A registration document e.g. certificate, issued by a registration authority e.g. NSW RMS, SafeWork, etc.  The registration document should clearly identify the registered plant usiing manufacturer, model and serial/VIN number, as a minimum.

SF06eSafety Check Document (Pink Slip)Australian state government regulators no longer typically issue "pink slips" required for registration of vehicles.  Most of this is done online and the output of the NSW system is an eSafety Check Document, other states may differ.  NSW RMS also defers this requirement for vehicles less than 5 years old. The intent is for the Plant Owner to provide documented evidence that the vehicle has passed an inspection that complies with the state's regulations for vehicle road registration.

SF07Brake Test Certificate / ReportFor road registered vehicles, this means a dynamic brake test certificate issued by a NSW RMS authorised inspector and in compliance with NSW RMS regulations for road registration. Where vehicles are registered in other states, then this means the equivalent certification provided in that state.
For non-road registered vehicles and plant, this means a dynamic brake test report completed by a competent and authorised person, which identifies the brake performance against specified acceptance criteria.  The report should include any printed output from the brake test equipment e.g. Milennium Brake-Testa.

SF09NHVR Compliance CertificationA formal document from a regulatory authority or representative that certifies that a road going medium or heavy vehicle (GVM>4.5 tonnes) complies with the requirements of the National Heavy Vehicle Regulations (NHVR).

SF10Brake Adjustment Report < 80% travelA brake adjustment report (or equivalent document) completed by a trained, competent and authorised mechanical tradesperson confirming the correct adjustment in accordance with OEM recommended practice and meeting specified acceptance criteria.

SF11Service / Maintenance HistoryThe Plant Owner must provide a summary of the inspections, services and maintenance activities carried out on the equipment.  The minimum acceptable is the last completed routine services and maintenance work for past 12 months. If the plant is less than 12 months old, then the maintenance activities since delivery of the plant.

SF12Engineering Drawings (General Arrangements as a minimum)Engineering drawings must be provided for the plant item. As a minimum this includes one or more General Arrangement drawings showing plant elevations and plan views.  Other relevant engineering drawings can be provided including assembly drawings, sectional views, detail drawings, etc.  The intent is that engineering drawings are available that are required for the safe operation and maintenance of the plant.

SF13Change Management HistoryA documented historical list of the modifications, alterations, and upgrades completed on the plant.  Where possible the reason for each change should be included. This includes upgrades and modifications issued by the OEM and any aftermarket modifications performed by the plant owner

SF14Operation Manual / InstructionsMeans a digital / electronic copy of an OEM manual that describes how to operate the plant correctly, the operational hazards, the applicable safety precautions, and operating instructions.  This could be provided individually or as part of a larger Installation, Operation and Maintenance Manual. The manual must clearly show that it is applicable for the model of the plant.

SF15Service / Maintenance ManualMaintenance or service manual supplied by the OEM for the specific make and model of plant

SF16Commissioning ReportA commissioning checklist and report that records the individual checks, set up parameters, and critical tests that were conducted before placing the plant into service. The MAC Mechanical and Electrical Commissioning Checksheets should be incuded as applicable.

SF17Thermographic Imaging ReportA thermographic survey report issued by a trained, competent and certified Level 1 thermography technician.  The report must include images and relevant temperature measurements.  Surface temperatures should not exceed 150 degrees C.

SF18Confined space identification / registrationWhere the plant has a space that is deemed a confined space in accordance with the Australian Standards, a confined space register should be maintained by the Plant Owner and a copy submitted for the plant safety file.

SF19Plant Design RegistrationSpecifically this is a copy of the certificate of design registration issued by SafeWork NSW, or an equivalent interstate regulatory authority.  An information plate fitted to the plant item should also clearly display the design registration number, so a photograph of this should also be provided.

SF20Plant Item RegistrationSpecifically this is copy of the certificate of plant item registration that is issued by SafeWork NSW or equivalent insterstate regulatory authority.  The certificate must be current and in date at the time inspection.  A compliance information plate is also required to be fitted to the plant with the plant item regstration number engraved or stamped.  A photograph of this plate should be submitted as proof that the plate was in place at the time of inspection.

SF21Pressure Vessel Test CertificateA certificate issued by certified pressure vessel inspector (AICIP PV Inspector) following completion of a statutory pressure vessel inspection in accordance with AS 3788.  The test certificate must be current at the time of inspection.

SF22NDT ReportA non-destructive test report (NDT) is a report issued by a trained and competent NDT technician as part of an ongoing asset integrity management program.  Separate NDT reports are usually issued for each test method used, so there may be multiple reports submitted.  All NDT reports must be issued with a NATA accreditation.

SF24Rim Register with OEM Test planThe plant owner of surface mobile plant or wheeled machinery with rims larger than 600mm (24 inch) must submit an extract from a rim register showing the identification of all rims fitted to the item of plant, and the inspection and test plan used to maintain integrity of the rims.  This could be a PDF file or a series of screen shots from a database, providing it meets the intent.

SF25CraneSafe Certification (Green Sticker)A CraneSafe Green Sticker is affixed to a mobile crane after it has been inspected by a third party assessor.  The Green Sticker is affixed to the mobile crane.  A photograph of the CraneSafe Green Sticker affixed to the crane is adequate evidence provided it is current at the time of inspection.  An inspection report from the assessor should also be submitted if available.

SF53Weighbridge CertificateA certificate or printed output from a vehicle weighbridge showing the weight of the vehicle, plant item, or transportable plant.  Unless advised otherwise the certificate or print out must show the empty vehicle weight (kerb weight) as a minimum.  For trailers it is required to weigh the trailer loaded to determine the gross trailer mass (GTM) and the aggregate trailer mass (ATM).

SF54Buoyancy AssessmentAn engineering calculation or design report issued by a professional engineer or OEM demonstrating the buoyancy of the floating plant, the amount of freeboard when loaded as designed, the loading conditions considered including wave height, wind speed, etc.

SF55Structural Engineers CertificateA certificate of compliance, or letter, issued by a registered professional structural engineer stating the structural load conditions for the design of the structure, and compliance to relevant Australian Standards for design of structures.

SF56Hydrostatic Leak Test ReportA test report issued by a competent engineer or certified inspector for hydrostatic leak tests conducted on the plant.  Typically, the report should show that the plant was able to hold a positive internal pressure (compressed air or nitrogen) for defined period of time without leakage.  This indicates that the plant is sealed and will not leak or taken on water.

SF57Stability Assessment /ReportMeans a stability assessment or report issued by an competent and authorised engineer.  The stability requirements and the approach vary significantly for different types of plant, but the outputs of stability assessments describe the limits of operation for the plant.
