There are two acceptable versions of this letter

The company name must match the name of the company registering

Invitations with an Accreditation expiry date of 01/07/2020 are still accepted but are no longer considered current in terms of Port Waratah Accreditation.

If your Invitation states that your current Accreditation is due for renewal on 01/07/2020, you must answer No to the  “Do you hold a current Contractor Accreditation with Port Waratah” question on the Certification page.

Business name on the certification is to match the organisation that is registered. If it doesnt match: Government certified evidence must be provided to link the ABN or business names together. This is ideally a letter of linkage or download from ABR showing the linkage

Must be certified AS / NZ 4801

JAS-ANZ stamp to be present on 4801 however not on the ISO 18001 - If a certificate is supplied that is a combined 4801 and 18001 there will be NO JAS-ANZ stamp present. This is acceptable.

From March 12, 2018: ISO 45001 will start to be issued in lieu of OHSAS 18001

From September 30, 2021: OHSAS 18001 will no longer be accepted - ISO 45001 MUST be supplied

• From Jul 13, 2023: AS/NZS 4801 will no longer be accepted as certified organisations are required to migrate to ISO 45001:2018.

Black and White or Colour is acceptable

NCIG SMS Audit Report

Business name on the audit report is to match the organisation that is registered. If it doesnt match: Government certified evidence must be provided to link the ABN or business names together. This is ideally a letter of linkage or download from ABR showing the

Port Waratah will be acknowledging the SMS Audits we do for NCIG.

If a company has a current NCIG SMS Assessment, they can upload the Audit Report our audit team issues from CMO and can then bypass the Port Waratah SMS.

It must be the full audit report, the company names must match (or linkage provided), it must clearly show the issue and expiry dates and it must be current.

• The subcontractors company name on the form must match the name of the company registering.
• The primary contractor listed on the form must be a registered Port Waratah company – they do not need to be compliant, just completely registered (incl subscription, documents, sms review).
• If primary contractor is NOT registered, leave application as pending – Do not return

• Expiry date to be entered as per the primary contractors accreditation date found on invitation to register
